XERF is a monopolar radiofrequency system from Cynosure Lutronic that combines two radiofrequencies — 2 MHz and 6.78 MHz — to heat tissue at more than one depth.
Most RF devices work at a single frequency, targeting one depth of tissue. XERF's dual-frequency approach reaches both the dermis (where collagen lives) and the SMAS layer — the same deep structural layer addressed in surgical facelifts — without any incisions.

XERF received US FDA 510(k) clearance K251327 on 11 August 2025. The applicant was Lutronic Corporation, and the device was found substantially equivalent to the Thermage FLX (K170758).
The cleared indication reads: “XERF is indicated for use in dermatologic and general surgical procedures for electrocoagulation and hemostasis.” Skin tightening comes from that controlled heating of tissue.
A 510(k) clearance is not an FDA “approval”, and the submission relied on bench testing against the predicate device rather than clinical trials. It tells you the device has been reviewed by a regulator. It does not guarantee a result. How XERF and Thermage compare.
Common, and expected: redness and warmth for a few hours; mild swelling, which can last a day or two; skin that feels tender to the touch for a few days.
Uncommon: small raised marks from the treatment tip, which settle within days; temporary tingling or numbness.
Rare, but reported with monopolar radiofrequency: burns or blisters, and very occasionally a small area of fat loss that leaves a slight dip in the skin. Careful energy settings and continuous skin cooling are how these are avoided.
Message the clinic on WhatsApp if you notice blistering, a spreading red area, or pain that is getting worse rather than settling.
Absolute contraindications: pacemakers, implantable cardioverter-defibrillators or any implanted electrical device — monopolar radiofrequency passes current through tissue and can interfere with them. Pregnancy.
Requires assessment before treatment: metal implants or permanent fillers in the treatment area; active skin infection or inflammatory disease at the site; uncontrolled diabetes affecting wound healing; history of keloid scarring; isotretinoin within the last six months; current or recent malignancy; severe uncontrolled cardiac disease.
These questions are asked before every XERF treatment at Elea Clinic, by the dermatologist rather than a therapist. Some patients are advised against treatment. A provider who does not screen for implanted electrical devices before applying monopolar radiofrequency is not treating safely.

Is XERF the same as HIFU?
No. HIFU (High-Intensity Focused Ultrasound) uses sound energy at single focal points. XERF uses dual-frequency monopolar radiofrequency that delivers energy simultaneously to two tissue depths — the dermis and SMAS — in a single pass. Different technology, different mechanism, different experience.
Is XERF FDA approved?
XERF is FDA 510(k) cleared (K251327, 11 August 2025), which is a different process from FDA approval. The cleared indication is “dermatologic and general surgical procedures for electrocoagulation and hemostasis”. The clearance was based on equivalence to the Thermage FLX, not on clinical trials.
Is XERF painful?
Most patients describe a deep, warm sensation during each pulse. The handpiece cools the skin surface throughout, and Cynosure Lutronic states that treatment does not normally need numbing. dr. Jesslyn adjusts the energy to your tolerance during the session.
How many sessions will I need?
