XERF vs HIFU: which is right for your face?

Both tighten skin without surgery. They deliver energy in fundamentally different ways, and that difference decides which one suits your face — assessed here by a board-certified dermatologist.

This is the most common question we are asked about skin tightening, and it deserves a straight answer rather than a sales one: neither is better. They do different things, and the right choice depends on your face.

Both are non-surgical lifting treatments. Both work by heating tissue below the surface so that collagen contracts and then remodels over the following months. The difference is how the energy is delivered, and therefore how it behaves in different kinds of skin.

Volumetric heating treats an area. Focal heating treats points. Faces that need overall firming and faces that need targeted lift are not the same problem.

Where HIFU is the better choice

We offer XERF and we will still say this plainly. HIFU tends to suit a patient with good skin thickness and a specific, focal area of descent — a defined jowl, a discrete brow drop — where depositing concentrated energy at fixed points is exactly what is wanted. It is also well established, widely available and generally cheaper per session in Bali.

If that describes you and budget is the deciding factor, HIFU is a reasonable treatment and you should not be talked out of it.

Where XERF has the advantage

What neither will do

This is the honest limit. Neither XERF nor HIFU is a facelift. If there is significant skin excess — if the tissue can be gathered between two fingers and there is a lot of it — energy-based tightening will produce a modest improvement and you may feel it was not worth the cost. A surgical opinion is the more useful conversation at that point, and we will say so.

Neither replaces volume. Laxity and volume loss look similar in the mirror and are different problems; a face that has deflated needs bio-remodelling or filler, not heat.

Can you have both?

Yes, but not casually and not close together. Tissue that has been treated with HIFU dissipates heat less efficiently afterwards, which raises the risk of thermal injury if radiofrequency is applied at standard settings too soon. If you have had HIFU, tell us when and where — energy and shot count are reduced accordingly. This is one of the specific cautions covered in the manufacturer’s advanced clinical programme.

Questions we are asked

Is XERF the same as HIFU?

No. XERF uses monopolar radiofrequency delivered volumetrically across a zone of tissue; HIFU uses focused ultrasound deposited at precise focal points. Both stimulate collagen, but they behave differently in different skin types and suit different patterns of laxity.

Which is more painful, XERF or HIFU?

Most patients who have had both describe XERF as more comfortable. HIFU is often described as a sharper, punctate sensation, while XERF is felt as deep warmth.

Which lasts longer?

Both peak at roughly two to three months and are typically repeated at intervals measured in months rather than weeks. Duration depends more on age, skin quality and lifestyle than on which device was used.

Can I have XERF if I have already had HIFU?

Usually yes, but the plan changes. Previously treated tissue dissipates heat less efficiently, so energy and shot count are reduced over those areas. Tell the clinic when and where you had HIFU.

Is XERF or HIFU better for a sagging jawline?

It depends on whether the laxity is diffuse or focal. A broadly softened lower face generally responds better to volumetric radiofrequency; a single discrete jowl may respond well to focal ultrasound. This is assessed at consultation.

Will either give me a facelift result?

No. Both produce a modest lift and improved firmness. If there is significant skin excess, energy-based tightening will underdeliver and a surgical opinion is the more useful conversation.
