XERF for Jawline and Jowls: Who Is a Good Candidate?

Four different processes can produce the same-looking jowl. Which one you have decides whether tightening is the right treatment, or the wrong one.

Short answer. XERF can tighten early-to-moderate jowling when the main problem is lax skin and soft tissue along the jaw. It is a weaker choice when jowls are driven mostly by a heavy fat pad, by bone loss along the jaw, or by advanced skin excess — those need other treatments, sometimes surgery. Because jowls usually have several causes at once, an examination is the only reliable way to decide.

What actually makes a jowl

Anatomists describe the jowl as multifactorial (Talei 2024). Four processes contribute in different proportions from person to person.

ProcessWhat happensWhat helps most
Soft-tissue and skin laxityConnective tissue around the mandibular ligament lengthens; skin and subcutaneous tissue become redundant (Minelli 2023)Energy-based tightening such as XERF if mild to moderate; surgery if advanced
Fat compartment changeJowl fat, the lowest subcutaneous compartment of the face, changes and descends; the face does not age as one mass (Rohrich & Pessa 2007)Depends on volume. A heavy fat pad is not a radiofrequency problem
Bone remodellingThe mandible loses height and length and the jaw angle widens with age (Shaw 2010)Structural support, for example filler along the jaw or chin by an experienced injector
Loss of support aboveMidface volume loss lets tissue slide towards the jawTreating the midface first can soften jowls more than treating the jaw

Who tends to be a good candidate

Published reviews of radiofrequency tightening report better results in younger patients with mild-to-moderate laxity (O’Connor 2021) and higher satisfaction among people seeking modest rejuvenation (Austin 2021). In practice, good candidates usually:

How a jawline is assessed

Clinical observation — not published evidence

The assessment is done sitting upright with the head in a neutral position, because lying down changes how the jaw looks. dr. Jesslyn looks at how much skin moves when gently lifted, how much fat sits in front of and below the jaw, whether the chin and jaw angle are well supported, whether the midface has lost volume, and how the two sides compare. She then explains which of the four processes are driving that face before recommending anything.

After more than 100 XERF treatments, the clearest jawline responses have been in early-to-moderate laxity in patients whose chin and jaw are well supported. Where jowls are mostly fat or bone-related, she will often recommend treating that first, or not using XERF at all. Asymmetry is the norm: one side of the jaw frequently needs more energy than the other, which is why treatment is planned per side.

Is XERF better than filler or threads for jowls?

They solve different problems. XERF tightens soft tissue; filler restores structure and definition along the jaw or chin; threads reposition tissue mechanically for a limited time. Many people need one of these, and some benefit from a sequence. We set out realistic non-surgical options on our non-surgical facelift page, and compare energy devices in XERF vs HIFU.

Who should not have XERF

Not treated: anyone with a pacemaker, an implantable defibrillator or another implanted electrical device — monopolar radiofrequency passes current through tissue and can interfere with them. Pregnancy.

Assessed carefully first: metal implants or permanent filler in the treatment area; active skin infection or inflammatory skin disease at the site; a history of keloid scarring; isotretinoin within the past six months; current or recent malignancy; poorly controlled diabetes.

Probably better served by something else: heavy skin excess, where a surgical result is more meaningful than a tightening treatment. The full list sits on the XERF safety section.

Questions we are asked

Can XERF get rid of jowls?

It can reduce early-to-moderate jowling by tightening lax tissue. It does not remove fat pads or replace bone, and it does not reproduce a surgical lift.

How many sessions for the jawline?

Usually one, reviewed at about three months. Some patients benefit from a second.

At what age is XERF best for the jawline?

Tissue matters more than age. People in their late thirties to fifties with early-to-moderate laxity often do well, but skin quality and anatomy decide.

Should I have XERF or a facelift?

For early change, XERF is less invasive with minimal downtime. For advanced laxity, surgery gives a larger and longer-lasting result. A dermatologist should tell you which category you are in.
