“Anti-ageing” is a marketing word covering at least four separate problems. A face changes over time through volume loss, laxity, surface quality and pigment — and they need different treatments. Treating the wrong one is the most common reason people spend money and see very little.

Most disappointing anti-ageing results are not a bad treatment. They are a good treatment aimed at the wrong problem.

Most faces over forty are doing two or three of these at once. That is why a single treatment rarely satisfies, and why sequencing matters more than choosing.

A dermatologist’s approach, by decade

Thirties — protect and maintain

Collagen production has begun to decline but structure is intact. This is the cheapest decade to treat, because prevention costs less than correction. Sun protection is not a throwaway line here — in Bali’s UV index it is the single highest-value intervention available. Beyond that: skin boosters for quality, targeted botulinum toxin for lines that are starting to etch in at rest, and pigment managed early before it consolidates.

Forties — the structural decade

Laxity usually becomes the dominant complaint. This is where energy-based tightening earns its place, and where under-dosing is the common failure. Volume loss often begins concurrently, so combining structural tightening with bio-remodelling tends to read as “well” rather than “done”.

Fifties and beyond — realistic combination

Multiple processes are active at once and expectations need to be honest. Combination planning over months, rather than one large intervention, gives the most natural result. This is also the decade where a surgical opinion is sometimes the more useful answer, and we will say so when it is.

Sequencing: what order, and why

  1. Diagnose pigment first. Several devices worsen melasma. Establishing what the pigment is before applying energy prevents an expensive setback.
  2. Structure before surface. Tightening changes how the face sits; treating surface quality first means re-treating afterwards.
  3. Leave intervals. Recently injected areas have altered circulation, which changes how energy-based treatments must be dosed over them.
  4. Judge at three months. Collagen remodelling continues for weeks after a session. Assessing at two weeks and concluding it failed is the most common mistake patients make.

Who should not have energy-based tightening

Monopolar radiofrequency is generally avoided with a pacemaker or other implanted electronic device, during pregnancy, over active infection in the treatment area, and over the thyroid and vocal cord region. Energy is reduced over recently injected areas and over tissue previously treated with HIFU or liposuction. Melasma requires a modified approach. Suitability is assessed individually.

What are the best anti-ageing treatments in Bali?

There is no single best treatment, because ageing is at least four separate processes: laxity, volume loss, surface quality and pigment. Laxity responds to energy-based tightening such as XERF; volume loss to bio-remodelling or filler; surface quality to skin boosters and laser; pigment to targeted pigmentation treatment. A consultation establishes which apply to you.

At what age should I start anti-ageing treatment?

Prevention in the thirties costs less than correction later, and in Bali's UV conditions sun protection is the highest-value intervention at any age. There is no correct starting age, but treating a problem early is generally cheaper than treating it once established.

How long do anti-ageing treatments last?

Energy-based tightening is typically repeated at intervals measured in months. Injectable bio-remodelling is usually a course followed by maintenance. Results depend on age, skin quality, sun exposure and lifestyle.

Can I combine treatments in one visit?

Sometimes, but sequencing matters. Recently injected areas change how energy-based treatments must be dosed, and pigment should be diagnosed before certain devices are used. A plan is built in order rather than all at once.

Will anti-ageing treatment make me look done?

Not if it is dosed to preserve expression and movement. The aim at Elea Clinic is that the face reads as well rather than treated.

When should I consider surgery instead?

When there is significant skin excess. Energy-based tightening produces modest improvement; if the tissue can be gathered generously between two fingers, a surgical opinion is the more useful conversation and we will say so.
